Problem's Popping up everywhere!

I've installed the following mods : polls, user post stats, and a few others,

But about a couple months later, some of my users where getting logged out, deleted, etc. Including me and I am the admin!

What do you guys think? Anyone heard of this before?

sounds like some one hacked you or your server is messed up
